Our Mission:

Above all we strive to give our clients an improved quality of life. It is the goal of this program that every individual that attends:

• Be treated with respect
• With assessment, monitoring, medication management, and physician updates by the nurse will at the least maintain health status when possible
• Will not only  be provided “proper nutrition” but will enjoy their meals
• Will be able to make their own choices in all things possible
• Enjoy holidays even if they have no one outside of this program
• Will, with support, remain in present living situation

Who We Serve

Medicaid recipients and private pay clients of a wide range of abilities and ages. Our clients typically show needs in these areas:

• ADLs (Bathing, ambulating, eating)
• Getting to/scheduling/keeping appointments
• Maintaining a healthy blood sugar
• Maintaining healthy blood pressure
• Eating a balanced diet
• Chronic wounds
• Disruptive behaviors requiring supervision
• Managing Medications
• Ambulation
